How to Join Longhorn EMS

What is the RIS?

The Registration Information Sheet (RIS) is a required application that must be submitted by the designated deadline to be a member of Longhorn EMS. The RIS asks for basic demographic information, EMS Licensure Status, and Immunizations. The RIS must be submitted every academic year and opens up at the beginning of the Fall and Spring Semester. 

Interested in joining Longhorn EMS? Membership dues for the 2025–2026 school year are $45 for the whole year or $30 per semester.

Our Fall '25 RIS is now OPEN and due by Friday, September 19th at 11:59pm.

Applications are ROLLING and priority for events will be given to those who submit early!

The RIS will need to be submitted for ALL members regardless of previous status.

NOTE: The deadline to submit your RIS is for an initial submission. If you have not received all immunizations or are waiting on licenses, you can always update your RIS after the initial submission.

Demographics

  • Full Name 

  • UT eid

  • Email 

  • Phone Number

  • Gender

  • Hometown

  • Graduation Date

  • Headshot (for ID badge)

Certifications/Licenses 
(if necessary, not required)

  • Texas EMS License Number and Expiration

  • AHA BLS/CPR Certification Number and Expiration 

  • FEMA 100 [Responder 1]

  • FEMA 200 [Responder 2]

  • FEMA 700 [Responder 2]

  • FEMA 230 [Responder 3]

Immunizations

  • Full UT Immunization Record 

  • Most Recent Influenza Vaccine Record*​​

  • COVID-19 Primary Series*

  • FOR RESPONDERS: Tuberculosis Test 

    • Must be either a Tuberculin Skin Test (TST) or a TB Gold Test. TB test must have been administered no more than 1 year prior to joining Longhorn EMS

*Additional Information* 

New Members OR Inactive Spring '25 members will be asked to submit a Personal Statement that will allow us to better get to know prospective members.

Longhorn EMS uses Discord to communicate and stay in touch with all members. Once you enter, please change your profile name to your First and Last name so that we can add you to the proper channels. If you get removed from the server, please email longhornemsaustin@gmail.com and we will add you back.
